Conditions Treated by an Oculofacial Plastic Surgeon

IN SAN DIEGO

At Oculofacial Surgical Arts in Del Mar, we specialize exclusively in cosmetic and reconstructive surgery of the eyelids, brows, and surrounding facial areas. Many patients come to us because of concerns around their eyes that affect both appearance and function. Below, you’ll find common conditions patients seek treatment for, along with modern surgical and non-surgical solutions.

Hooded Upper Eyelids

Excess skin can create a “hooded” or heavy look, often making patients feel older or more tired. In some cases, vision can even be impaired.

Treatment: Upper eyelid blepharoplasty to remove excess skin, restore a natural eyelid crease, and brighten the eyes.

When the upper eyelid margin sags and covers part of the pupil, it can obstruct vision and create a sleepy appearance.

Treatment: Ptosis repair surgery to tighten the eyelid muscle and lift the eyelid into its proper position.

Under-Eye Bags

Puffy under-eyes caused by protruding fat pads can make you look perpetually tired.

Treatment: Lower eyelid blepharoplasty with fat repositioning to smooth the under-eye contour and restore youthful definition.

Abstract Design

Dark circles can result from hollowing, pigmentation, or thin skin.

Treatment: Options may include lower eyelid blepharoplasty with fat repositioning, laser resurfacing, PRP therapy, or topical treatments depending on the cause.

Under-Eye Hollows (Tear Troughs)

Hollowing beneath the eyes can cast shadows that create a tired or aged appearance, even when you feel well-rested. This is often due to natural fat loss, genetics, or changes in the midface with age.

Treatment: Lower eyelid blepharoplasty with fat repositioning can smooth and restore youthful volume and blend the eyelid–cheek junction. Unlike temporary fillers, which may lead to swelling or irregularities, surgical fat repositioning provides a long-term and often permanent improvement.

Fine lines, crow’s feet, and crepey texture often appear around the eyes because the skin here is the thinnest on the face. Sun damage, collagen loss, and repeated facial expressions (like smiling or squinting) make these changes more noticeable with age.

Treatment: COâ‚‚ laser resurfacing precisely targets delicate eyelid skin to smooth wrinkles, tighten crepey areas, and stimulate new collagen. This treatment can be performed alone or combined with eyelid surgery for enhanced results.

Wrinkles When I Smile

Dynamic wrinkles that appear only with expression are often treated non-surgically.

Treatment: Neuromodulators, such as Botox, to soften lines while preserving natural expression.

Abstract Design

Sagging brows can weigh down the eyelids, exaggerating hooding and making the eyes appear tired or sad.

Treatment: Endoscopic or lateral/temporal brow lift to elevate the brows and open the eyes naturally.

Unhappy with Appearance After Eyelid Surgery

Some patients feel their prior eyelid surgery left them with unnatural results or functional concerns.

Treatment: Revision eyelid surgery tailored to restore symmetry, improve aesthetics, and address scarring or rounding.

Swelling or bulging of the midface and lower eyelids (festoons) can be difficult to treat. This may result from fluid retention, aging changes, or prior treatments.

Treatment: A detailed evaluation is performed; options include laser resurfacing, direct surgical excision, or advanced lower eyelid techniques depending on severity.

Under-Eye & Cheek Swelling (Festoons & Malar Edema)

Swelling After Under-Eye Fillers

Persistent puffiness or migration of filler under the eyes can leave patients looking worse, not better.

Treatment: Hyaluronidase (filler dissolving enzyme) followed by either surgical or non-surgical rejuvenation to restore a natural contour.

Abstract Design

Lower eyelid malposition or scarring can lead to a rounded eye appearance.

Treatment: Lateral canthoplasty to restore the almond eye shape and improve eyelid position.
