Filler Dissolving
IN SAN DIEGO
Oculofacial Surgical Arts offers ultrasound-guided filler dissolving in San Diego with Dr. Jean-Paul Abboud. Using high-frequency ultrasound to locate retained hyaluronic acid filler, Dr. Abboud guides injections of hyaluronidase into identified deposits.
Dermal fillers can restore volume and soften facial hollows. Many widely used products, including the Juvéderm® and Restylane® families, contain hyaluronic acid, a water-attracting substance formulated into a gel that adds fullness beneath the skin.

Filler does not address every cause of facial aging. Prominent lower eyelid fat, excess skin, and tissue laxity may require a different approach. Adding more filler can sometimes conceal the underlying problem while creating additional fullness.

Recognizing filler complications
• Persistent or recurrent swelling and puffiness.
• Lumps, ridges, asymmetry, or uneven contours.
• Excess fullness or an overfilled appearance.
• Blue-gray discoloration beneath thin skin.
• Migration beyond the intended treatment area.
• Delayed inflammation, nodules, or infection.
Problems may develop shortly after injection or become apparent months to years later. Rarely, filler can enter a blood vessel and cause serious injury, including skin damage, blindness, or stroke.
When added volume becomes an unwanted change.
01 / UNDERSTANDING FILLERS

The tear trough is the hollow near the inner lower eyelid where the eyelid meets the cheek. Although filler may soften this depression in selected patients, the lower eyelid has exceptionally thin skin, limited space, and delicate fluid-drainage pathways.
Persistent puffiness & malar edema
Hyaluronic acid attracts water. Retained filler and its effect on surrounding drainage may contribute to under-eye swelling or fluid accumulation over the upper cheeks, known as malar edema. This can occur even after an initially satisfactory result.

Blue-gray discoloration
Superficially placed filler can create a bluish hue, commonly called the Tyndall effect. It may resemble dark circles, even when the issue is filler placement rather than skin pigmentation.
Lumps & an uneven eyelid-cheek transition
Small deposits and contour irregularities can show through thin eyelid skin. Excess volume may blur the natural transition between the eyelid and cheek, creating heaviness rather than a refreshed appearance.
Not all under-eye swelling is caused by filler. An examination helps distinguish retained product from prominent fat pads, skin laxity, festoons, inflammation, and other medical causes.
02 / UNDER-EYE CONCERNS
Why under-eye filler deserves special attention.
Hyaluronic acid fillers are biodegradable, but they can persist much longer than many patients expect. Product characteristics, injection location, and individual factors influence how long the material remains.
Some studies have detected residual filler years after the last reported injection, including approximately 15 years later. Even injections performed many years ago should therefore be included in your treatment history. Repeated touch-ups can add volume to areas where filler remains, making it important to assess what is still present before deciding whether to inject more.

03 / FILLER LONGEVITY
The visible result may fade. The filler may remain.

Ultrasound-guided filler dissolving combines high-frequency imaging with targeted hyaluronidase injections. Ultrasound uses sound waves, without radiation, to provide information about filler and the surrounding tissues.
An external examination shows your facial contour. Ultrasound adds information about the location, depth, and distribution of identifiable filler deposits and their relationship to nearby anatomy and blood vessels.

Locate
Assess identifiable filler deposits, including areas where product may have migrated or remained across multiple tissue layers.

Target
Use real-time imaging to guide hyaluronidase into the deposits selected for treatment.
Reassess
Evaluate your clinical response and use follow-up imaging when helpful to assess suspected residual filler.
Ultrasound can be particularly useful when treatment records are incomplete or previous dissolving has not produced the expected improvement. It cannot guarantee identification of every trace of filler, establish every product’s exact brand, or eliminate treatment risks. Imaging is interpreted alongside your history and examination.
04 / ULTRASOUND GUIDANCE
See beneath the surface. Treat with greater precision.
Hyaluronidase is an enzyme that breaks down hyaluronic acid, allowing the body to clear the treated material. During ultrasound-guided treatment, it is injected into identified deposits contributing to unwanted fullness, swelling, or irregularity.
Treatment is tailored to the location and extent of the filler, its response, and your goals. Some patients improve after one session; others require staged treatment. Complete removal and full resolution of swelling cannot be guaranteed.
Hyaluronidase works on hyaluronic acid filler. It does not dissolve Radiesse® (calcium hydroxylapatite), Sculptra® (poly-L-lactic acid), silicone, or transferred fat. Using hyaluronidase for cosmetic filler dissolving is an off-label use in the United States.

05 / HOW DISSOLVING WORKS
A targeted approach to dissolving hyaluronic acid filler.
Previous under-eye and cheek filler matters when planning lower blepharoplasty, even if the injections were performed years ago.
At Oculofacial Surgical Arts, Dr. Abboud’s approach is to dissolve retained hyaluronic acid filler in the planned lower eyelid treatment area before surgery, then allow the tissues to settle before final surgical planning.
Reveal your underlying anatomy
Filler and associated swelling can conceal hollows, prominent fat pads, and the natural eyelid-cheek transition. Dissolving helps distinguish filler-related fullness from the anatomy that surgery needs to address, including planning for fat repositioning.
Clarify the surgical planes
Surgical planes are the natural tissue layers through which a surgeon works. Retained filler and associated tissue changes can distort these layers and make surgical landmarks and fat compartments less clearly defined.
Address a source of prolonged edema
Residual filler may contribute to persistent swelling after lower eyelid surgery. Treating filler-related concerns beforehand helps establish a more reliable baseline, although dissolving cannot prevent all postoperative swelling or guarantee that existing edema will resolve.
The interval between dissolving and surgery is individualized. Dr. Abboud reassesses your anatomy and the stability of the tissues before proceeding.
06 / BEFORE EYELID SURGERY
A clearer starting point for lower eyelid surgery.

Consultation & examination
Discuss your concerns, previous injections, and any prior dissolving. Bring treatment records when available, including product names, approximate amounts, injection sites, and dates. Dr. Abboud assesses whether retained filler is contributing to the problem.
Ultrasound assessment
The areas of concern are examined with ultrasound to locate identifiable deposits and evaluate nearby anatomy. Findings guide the discussion about whether dissolving is appropriate.
Targeted treatment
If indicated, hyaluronidase is injected into selected areas under ultrasound guidance. Brief stinging or pressure may occur. Your treatment plan and risks are reviewed beforehand.
Follow-up & reassessment
Your response is evaluated as the tissues settle. Further treatment may be considered if residual filler continues to contribute to symptoms or unwanted contours.
Recovery and results
Temporary swelling, bruising, redness, and tenderness can occur. Hyaluronidase begins acting promptly, but early swelling can obscure the result. Elective treatment may be reassessed after approximately 48 hours; the appearance can take around two weeks or longer to evaluate when swelling is significant.
As volume decreases, underlying hollows, skin laxity, or asymmetry may become more apparent. Unexpected or persistent changes should be evaluated. Your aftercare and follow-up schedule are individualized.

What to expect at your appointment.

Does hyaluronidase dissolve healthy tissue, fat, or collagen?
Hyaluronidase breaks down hyaluronic acid; it does not directly dissolve fat or collagen. Describing it as an enzyme that melts healthy tissue misrepresents its mechanism. However, it can affect naturally occurring hyaluronic acid as well as injected filler. Your body continually produces and renews its natural hyaluronic acid. Experimental research has not demonstrated significant damage to skin structure or collagen-producing cells under the conditions studied, but these findings do not prove that every treatment is free of risk or explain every reported post-treatment change.

Can dissolving under-eye filler cause hollowing or loose skin?
Removing filler removes the volume it provided. Preexisting hollows, laxity, and changes associated with aging may become more visible. Patients have also reported unwanted hollowing, wrinkles, and skin-quality changes after dissolving. These concerns deserve assessment; their occurrence alone does not establish that hyaluronidase destroyed fat or collagen. Research into these outcomes remains limited.

Is ultrasound-guided filler dissolving safe?
Treatment has risks, including swelling, bruising, pain, redness, incomplete correction, unwanted contour changes, and allergic reactions that can occasionally be serious. Ultrasound adds information and helps guide placement, but does not eliminate risk. Your history, allergies, prior reactions, and goals are reviewed before treatment.

How long does filler dissolving take to work?
The enzyme begins acting promptly, and changes may become noticeable over the following days. Early swelling can hide the result. Assessment may begin around 48 hours, while the appearance can take approximately two weeks or longer to settle. The timeline varies with the filler and the degree of swelling.

How many dissolving sessions will I need?
There is no fixed number. Some patients improve after one session, while others require repeat treatment because of the amount, distribution, or response of the filler. Persistent swelling alone does not prove that additional dissolving is needed.

Can under-eye filler still be dissolved years later?
Yes. Retained hyaluronic acid filler may respond to hyaluronidase years after injection. The age of the injection alone does not rule out treatment, although the response and number of sessions vary.
08 / YOUR QUESTIONS, ANSWERED
Your questions, answered: safety & recovery.
Can you treat migrated filler or filler from another provider?
You can schedule an assessment even if your filler was placed elsewhere. Ultrasound can help locate identifiable deposits, including outside the originally intended area. Hyaluronic acid filler may be treated with hyaluronidase; other materials require a different approach. Previous treatment records are helpful.

Can ultrasound find all filler and identify its brand?
No. Ultrasound may identify deposits and their relationship to surrounding tissues, but it cannot reliably detect every trace or always establish the exact product. A normal or uncertain scan does not replace a clinical assessment.

Can only part of my filler be dissolved?
Selective reduction may be possible, but it is not perfectly predictable. Hyaluronidase can spread beyond the injection point, so preserving an exact amount or shape of nearby filler cannot be guaranteed.

Why am I still swollen after filler dissolving?
Possible explanations include temporary injection-related swelling, residual filler, inflammation, persistent drainage changes, or a concern unrelated to filler. Some patients continue to have malar edema even after apparent filler removal. Reassessment helps determine whether more dissolving or another approach is appropriate.

Will dissolving filler remove eye bags or festoons?
It may improve fullness caused by filler or associated fluid retention. It does not directly correct prominent orbital fat, excess skin, or every cause of festoons. An examination helps identify which concerns are likely to improve and whether other treatment is appropriate.

Do I need filler dissolved before lower blepharoplasty?
At Oculofacial Surgical Arts, Dr. Abboud’s approach is to dissolve retained hyaluronic acid filler in the planned lower eyelid treatment area before surgery. This helps reveal the underlying anatomy, address filler that can obscure surgical planes, and treat a potential source of prolonged edema. The final plan is based on your examination and treatment response.
Your questions, answered: filler correction.
How long should I wait before lower eyelid surgery?
There is no single waiting period for every patient. Surgery is planned after the response to dissolving has been assessed and the tissues have settled enough for reliable planning. Persistent swelling or additional treatment may require more time.
Does dissolving hurt, and is there downtime?
You may feel brief stinging or pressure during the injections. Swelling, tenderness, redness, and bruising can follow. The amount of social downtime varies, especially around the eyes. Plan around the possibility of visible swelling or bruising and follow your individualized aftercare instructions.
How much does ultrasound-guided filler dissolving cost in San Diego?
Cost depends on the areas assessed, treatment complexity, medication required, and whether additional sessions are needed. Contact Oculofacial Surgical Arts for current consultation and treatment pricing. Cosmetic filler correction is generally self-pay; any potential insurance coverage depends on the circumstances and your plan.
Can I have treatment if I do not know which filler was used?
Yes, you can schedule an evaluation. Treatment records are helpful because not all materials respond to hyaluronidase. Ultrasound may provide clues about the material and its location, but it cannot always determine the exact product.
Can filler dissolve naturally or be removed at home?
Hyaluronic acid fillers can break down naturally, but the timeline is unpredictable and may be much longer than expected. There is no reliable at-home method for safely dissolving injected filler. Do not inject dissolving products yourself or aggressively massage a swollen area without medical guidance.
Can I get filler again after dissolving?
Future treatment should be considered after the tissues settle and the original problem is understood. Replacing under-eye filler may recreate the concern if it contributed to persistent swelling or an unnatural contour. Dr. Abboud can discuss whether observation, skincare, resurfacing, or surgery better addresses your anatomy.
